Crypto theft rose 55% quarter-on-quarter to $1.18 billion in the third quarter of 2026, across 107 incidents, according to blockchain security firm Hacken. Smart-contract exploits accounted for 48% of losses, while access-control and operational failures made up 40%. Losses linked to compromised keys and operational failures fell about 30% from the previous quarter. The 40% figure refers to the share of total third-quarter crypto hack losses attributed to access-control and operational failures, not 40% of incidents. Hacken’s report puts this at roughly $472 million of the $1.18 billion stolen.
